Camp Manito-wish YMCA Leadership Programs
Understanding of Risks, Acceptance of Responsibilities
Please read the following statements carefully. Then date and e-sign this form indicating that you have read and understand it. Participants under the age of 18 must have a parent or guardian e-signature.
- I accept the fact that, while the program leaders are skilled and experienced, they cannot guarantee my total safety since some risks are beyond their control. I understand that I will be exposed to risks of nature and to elements over which neither Camp Manito-wish YMCA nor its employees have any control.
- I agree to follow all instructions and guidelines given by Camp Manito-wish staff, and to act in a safe and responsible manner toward all participants.
- I will not use equipment be present on the ropes course or any body of water without Camp Manito-wish staff YMCA present.
- I agree to notify Manito-wish staff of any changes to my health and fitness that may occur during programming.
- I fully comprehend and willingly assume the responsibilities and risks of participating in this program as explained to me by the Camp Manito-wish YMCA staff.
- I give and grant Camp Manito-wish YMCA permission to use pictures of myself for promotional purposes. Professional and candid photographs and video of participants may be taken at camp and on the trail. These photos may be used for publication in promotional materials including the Camp Manito-wish YMCA Web site. Please let us know, in writing, if you have any objections. Participants may submit photographs for possible inclusion.
- I understand that I am responsible to my group leader to share any needed medical information as requested for participation in the program.
CODE OF CONDUCT
I UNDERSTAND THAT:
- I have made a commitment to attend the RYLA Conference and will notify the RYLA Conference Committee immediately if conflicts arise.
- I may not arrive late or leave the conference earlier than the conference's program schedule.
- I am responsible for attending all meetings and activities with my small group.
- I must sleep in the cabin to which I am assigned and respect lights out at the scheduled time. I will be quiet thereafter.
- There are separate sleeping areas for males and for females. For the privacy and security of all concerned, males ARE NOT allowed in female sleeping areas and vice versa.
- Portable radios, cassette players, and/or CD players are permitted only during scheduled recreational times. Items used during unauthorized times will be confiscated and returned at the end of the conference
- Recreational activities are limited to those periods of the day scheduled for such activities.
- If any person is injured or becomes ill, I will not move him/her. I will immediately contact a camp staff member or a Rotarian.
- I will report any damage or breakage immediately to my facilitator, cabin supervisor or any Rotarian. All participants are responsible for taking care of the camp facilities.
- Smoking is prohibited out of respect the health of all participants and the potential fire hazard in the area.
- Any medications, whether prescribed or "over the counter", must be identified on the container. Unidentified medications found in possession of any participants will be confiscated.
- Alcohol, weapons, and illegal drugs are prohibited. Any of these items found in possession of any participant will be immediately confiscated. In addition, the participant will be immediately dismissed from the Conference and reported to the proper law authorities.
- Foul or abusive language (including anything discriminatory or harassing in nature) will not be tolerated nor will physical violence or threats of physical violence.
- Any sexual or lewd misconduct by and/or between myself and other participants will be considered unacceptable behavior.
- I MAY NOT leave the camp area for any reason without the permission of a Rotarian.
- Confidentiality is important. Anything I have shared or heard within my group should remain in that group. However, I understand that camp officials are mandated by law to report any suicide or abuse issues to the proper authorities.
- I will respect the environment of the camp and surrounding area and will do all in my ability to protect it for future generations.
- I am to conduct myself in all sessions and activities, programmed or recreational, and in all facilities in a manner creditable to my school, Rotary International, my family, and myself, and will not cause injury to another person.
I have read the Code of Conduct and do hereby agree and commit to honoring them. Should my conduct be considered unacceptable at any time at the discretion of Conference officials, or should I transgress any of the codes stated above, I understand that I will be dismissed from the Conference and will be sent home at my parent's or guardian's expense.
