Durable Medical Equipment Program

Procedure

Procedure for Durable medical equipment program: Hospital Maintenance Department 

  1. Contact Plant Operations for drop off of durable medical equipment from Neighbor to Neighbor customers or donors at DCMH.
    a.   Maintenance staff – 743-5566 Ext. 3098
    b.    Director of Plant Operations-Daniel J. Boes --- 746-3597
  2. Delivery address and hours for equipment drop off at DCMH:
    1. Monday – Friday 7:00am-4:00pm  @ 330 South 16th Place – Service entrance and Maintenance office.
    2. Weekend drop off can be scheduled by calling Neighbor to Neighbor @ 743-7800.        
         
  3. Tagging system:
    1. If equipment has a Neighbor-to-Neighbor identification number attached no paper work needs to be completed by maintenance staff.
    2.  If equipment does not have a Neighbor-to-Neighbor identification number maintenance staff will ask the donor to complete a form with donor information.
    3. Maintenance will then contact Neighbor to Neighbor and ask for the next number to be assigned to this piece of equipment. (743-7800).
  4. Maintenance will inspect and repair equipment as appropriate. All equipment will be accepted at DCMH.
    1. If equipment is easily repaired it will be cleaned and put into system.
    2. If equipment is not repairable maintenance staff will discard that equipment.
  5. Maintenance department would then contact Eric VanAlstine in Housekeeping informing him that equipment is ready to be cleaned, or power washed.
  1. When equipment has been cleaned Eric VanAlstine will contact Neighbor to Neighbor and set up a delivery schedule for equipment. If Neighbor to Neighbor is unable to accept additional equipment Eric will contact yet to be determined Rotary membership contacts for disposition of overstocked equipment.
  1. After delivery schedule has been established Eric VanAlstine will contact Daniel J. Boes @ Ext.- 3597 to confirm delivery of equipment by maintenance department to Neighbor to Neighbor.
